11-facility SNF sold in Alabama

A Florida-based owner sold an 11-facility nursing home portfolio in Alabama totaling 1,205 beds, according to Forest Healthcare Advisors, which represented the transaction.

The portfolio was cash-flow positive and maintained strong occupancy levels at the time of the sale, while still offering opportunities for operational and financial growth. The buyer, an East Coast health care operator, expanded its existing footprint of skilled nursing and assisted living facilities through the acquisition.

Financial terms of the transaction were not disclosed.

8-facility SNF portfolio changes hands

An eight-facility skilled nursing portfolio totaling 1,111 licensed beds changed hands in Massachusetts, with assets concentrated in the Boston and Springfield metropolitan areas.

The portfolio included eight skilled nursing facilities, with 938 beds, or 84% of the total licensed capacity, located within the two metro markets, according to a press release issued by Blueprint, which managed the sale. The assets offered significant market share in a state known for high barriers to entry due to its certificate-of-need requirements and established referral relationships with major health systems, including UMass Memorial Health, Baystate Health and Tenet Healthcare.

At the time of the sale, the portfolio was recovering from post-pandemic occupancy and staffing challenges, with census levels trending upward and a path toward further stabilization. The state’s transition to the Patient Driven Payment Model (PDPM) in October 2023 was viewed as a potential catalyst for administrative cost reductions and future reimbursement improvements. Pro forma projections called for $121.9 million in revenue and $10.4 million in EBITDAR at 81% occupancy.

The portfolio attracted interest from multiple investors and operators drawn to its scale, protected market position and operational upside. No single facility accounted for more than 19% of consolidated revenue, providing diversification, while several flagship properties offered opportunities for future value creation.

The buyer, an established owner-operator, completed the acquisition following final approval from the Massachusetts Department of Public Health on June 1. Financial terms of the transaction were not disclosed.

355-bed SNF changes hands in Massachusetts

Evans Senior Investments (ESI) announced the successful closing of Project Beacon, a portfolio of five skilled nursing facilities across Massachusetts.

The portfolio consists of five well-established skilled nursing facilities totaling 355 functional beds, primarily concentrated within the strategic Boston-Providence corridor, a press release noted.

While operating in receivership, the properties presented a significant turnaround opportunity, supported by average occupancy of 89%.

The portfolio was acquired by a growing regional owner-operator looking to strategically expand their footprint in the Northeast.

ESI represented the institutional seller and identified over $2.6 million in immediate expense synergies from outsourced departments. It showcased $4.35 million in additional Medicaid and Medicare reimbursement. Through this comprehensive approach, ESI drove competitive interest and secured a buyer equipped with the scale and regional focus to maximize the portfolio’s potential.

2-facility SNF portfolio sold in Chicago

A two-facility skilled nursing portfolio in the Chicago area sold for $20.9 million, or approximately $66,000 per licensed bed.

The portfolio includes Community Care Center, a 204-bed skilled nursing facility in Chicago, and The Terrace, a 115-bed skilled nursing facility in Waukegan, Illinois. The seller, a private investment firm, elected to divest the properties as it neared the end of its investment horizon and shifted its focus to other opportunities, according to Blueprint, which brokered the sale.

Community Care Center, built in 1970, benefited from an increase in resident admissions after a nearby 200-bed skilled nursing facility on Chicago’s South Side ceased operations. The influx of residents contributed to improved occupancy and financial performance beginning in the third quarter of 2025, positioning the facility as a cash-flowing operation at the time of sale.

The portfolio offered the buyer a combination of one of Chicago’s larger skilled nursing facilities and a complementary asset in the northern suburbs with stable occupancy.

The transaction attracted multiple acquisition proposals before the seller selected an Illinois-based private investment firm as the buyer. Financial terms indicate the deal closed at approximately $66,000 per licensed bed.
