AllyAlign, LeadingAge Partnership Brings New Value-Based Offerings to California

Value-based care management company AllyAlign Health last week announced it will offer two new co-branded Medicare Advantage plans to members of LeadingAge California, thanks to a partnership with the senior advocacy state group.

The plans — institutional equivalent special needs plan (I-SNP) Align Thrive, and chronic conditions special needs plan (C-SNP) Align Connect — will be underwritten by California licensed Knox-Keane Healthcare Service Plan Align Senior Care and serve long-term care residents living with complex, chronic health needs as well as those diagnosed with dementia.

Richmond, Va.-based AllyAlign partners with long-term care providers to set up Medicare Advantage I-SNPs, C-SNPs and dual-eligible special needs plans (D-SNPs). The company currently operates in more than 550 communities across 22 states.

The California chapter of LeadingAge participated in the design of these plans to benefit operator members and the residents in their care. Member enrollment begins next year.

Reedley, Calif.-based Palm Village Retirement Community anticipates improved access to its residents’ access to care, along with a decline in hospital admissions, according to Palm CEO David Reimer.

AllyAlign plans are “clear, straightforward” options to offer value-based care, LeadingAge California stated.

“The collaboration with LeadingAge California allows us to offer Medicare Advantage plans designed for senior living residents by senior living communities.,” Mark Price, CEO of AllyAlign, added in a statement.

“The ‘Plan Plus Care’ model is designed to provide value-based care to residents and can be tailored to meet specific provider needs,” Jeannee Parker Martin, president and CEO of LeadingAge California, said in the press release. “We look forward to improved health outcomes for older adults across our membership and beyond with the launch of Align Senior Care.”
